Any change in your health, especially if it comes on suddenly, can be overwhelming. That’s why we’re dedicated, not only to providing you with the highest standards of post hospital care, but with the information and reassurance you need to make decisions that are in the best interests of both your medical and lifestyle concerns.
With all the recent changes in insurance coverage and hospital reimbursements, you’re bound to have questions and feel out of control when it comes to decisions about your care. Our exceptionally trained hospital liaison and admissions professionals are here to thoroughly explain your options and what the next steps are along your path to recovery. We’ll help you research your insurance coverage and help you make informed decisions. We’ll never rush you into a decision, but will be here to make sure you have a seamless transition from hospital to home.
Many people find it helpful to pre-plan their post-surgical care. Hospital stays, even for complex surgeries, are increasingly shorter. That’s why many people opt to make a plan ahead of time for where they will go following surgery to recover and regain their strength before returning home. As part of this planning, you can reserve your private rehab suite in our Transitional Care Unit where you can recover in a private and gracious setting. We encourage you to learn more about our center and we welcome tours any time that is convenient for you or your family.
Of course, there are times, such as in the case of a heart attack, stroke or injury, when the need for skilled nursing services is thrust upon you or a family member without warning. And so we invite you to get to know us before you need us. You can schedule a tour at any time to see our facility, meet our caring staff and learn about our services for yourself or for a loved one.
For answers to your questions or additional resources about transitional care, skilled nursing, or any of our services, please complete this form and one of our Admissions professionals will be in touch with you as quickly as possible.
